to our athletes. The documents below should be of assistance in
helping you to train your athletes and prepare your athletes and
volunteers for local and state competitions.
Medical Forms
Special Olympics require that all athletes have a medical form completed
by a physician and on file with both the state and local chapters.
Physicals must be current within three years from the date of competition.

Volunteer Application Forms
All coaches and volunteers who serve in a direct supervisory capacity
with Special Olympic athletes must have a Volunteer Application
Form on file with both state and local chapters. You must also submit
a recent photo with your application. These are not necessary for
volunteers who are not directly responsible for the care of athletes.
